Q.H/16 documents on Text over IP for Rapporteur meeting next week

Attached is an initial rough pass at an Annex for H.460.9 to incorporate the
RTCP Reporting Extensions metrics.  My ASN.1 may be a little rusty, hence
apologies for any syntax errors.
