AW: H.225.0 v4 inconsistencies/errors found

I want to make it clear that I didn't intend to re-open the octet 3a
debate.  On the contrary, I was trying to point out that it was put to
rest a long time ago.  I just want to fix the inconsistency that Karl
noticed.
