[h323plus] Codec Selection in H.239 Extended Video channel

Currently only H.263 is supported with H.239. You need to add the
PluginCodec_MediaTypeExtVideo to the H.261 plugin codec definition to tell
h323plus to load it into the H.239 plugin factory.

  I was trying to make a call using H.261 codec and then I opened
ExtendedVideoSession. The session opens and closes immediately.
  I guess it is because the call was established using H.261 codec so it
returns from the library,
