With respect to 'errror' conditions or inconsistencies on the GK database when doing look-ups, it seems as though that would really be a policy issue. The usual behaviour of 'least astonishment' should be the default.

I agree that it should be a policy issue. An administrator might not want to delay the call setup by having to look up all aliases in its registration database before responding to an ARQ. I know that one GK vendor I spoke to at the interop had this concern. Plus, what's the danger in not having all aliases resolve to the same transport address? The caller gets a wrong number? Not like that hasn't happened before. I still like good old #4.
